I The Magician
We're not sure about this one. We considered Tung, as Miles' mentor, before assigning the eras. We also considered Illyan, but settled on him as the King of Swords. Leading candidates are Captain Negri and Emperor Ezar.
ceolorince suggests Ser Galen as the Magician.
Mike suggests that Galen may be too malevolent a figure for the Magician.

After further discussion and consideration, Gregor is an excellent King of Swords. Illyan is now a leading candidate for the Magician.

III The Empress
We're also not sure about the Empress. Leading candidates are Lisbet Degtiar (the right era in-world, but a bit out of place) or Princess Kareen in match to Ezar Vorbarra as The Emperor; or Elizabeth Naismith in match to Piotr Vorkosigan as The Emperor.

IV The Emperor
While it might make sense to use Gregor as The Emperor, two factors dissuaded us from that: First, that the Emperor is an imposer of order, sometimes to the extent of being a tyrant, where Gregor is more a maintainer and grower of order. Second, that Gregor is not as active during this Cycle as The Emperor should be.
We have yet to find another place in the deck for Gregor, which is unfortunate, considering his great importance. Or for Laisa.

XXI The World
We had originally set The World as Miles' children.
kellan pointed out that we didn't have the uterine replicator anywhere in the deck yet, which was a oversight that had to be addressed, since the uterine replicator is the piece of technology which drives nearly all the speculative societies of the setting.
Everyone seems to agree that The World is the first logical place to look, for a place to add the uterine replicator, either as foreground or background.
Especially if we've already got the cryochamber, we need the uterine replicator (the cryochamber is a matched pair to the uterine replicator, especially in light of CryoBurn). Also, the Vertigo deck connects the cards of Death and The World, giving them related astrological symbols, but I have not yet found a second source on this connection.

The uterine replicator as the world also forces Cycle 4 to be re-evaluated as all-encompassing time: before and after. Which is perhaps fitting since we've given it the traditional, outside-the-flow position usually occupied by The Fool. Also fitting to the all-encompassing nature of The World.
